Written permission to administer prescription and non-prescription medication at school must be obtained from both the student’s parent and physician. Read our policy about dispensing medication here.
• Permission to Administer Medication Form (must be updated each school year).
This includes ALL medications including epinephrine auto-injectors, asthma inhalers, and antihistamines. Any medications received without the completed Permission to Administer Medication form (with parental and physician signatures) will be returned to the parent.
