Medical Forms Due - October 1st
All students in Kindergarten, 6th grade, and new to District 35 must be in compliance with the state’s minimum requirements. Proof of immunization must be submitted to the school nurse with the physician and parent signed State of Illinois Certificate of Child Health Examination Form by October 1, 2020. In accordance with state law, a student that is not in compliance with these mandates must be EXCLUDED from school beginning on October 15th until proof is obtained.
• All students entering Kindergarten and new to the district are required to submit a Vision Exam Form prior to October 1, 2020. This exam should be completed within 12 months of the October 1st deadline.

Minimum Immunization Guidelines

Proof of immunization must be submitted to the school nurse with the physician and parent signed State of Illinois Certificate of Child Health Examination by October 1st. Parents or legal guardians who are requesting a religious exemption to immunizations or examinations must use this form for students entering kindergarten, sixth, or ninth grade. This form may not be used for exemptions due to personal or philosophical reasons. Illinois law does not allow for such exemptions.
