PHYSICAL AND IMMUNIZATIONS
All students entering Kindergarten and Sixth grade are required by the state to have a current physical examination completed on the approved Illinois Department of Public Health Form. A sports physical or a camp’s physical does not meet the requirement. A current physical is one that was completed within one year of the starting date of the school year. The parent portion on the upper back side of the form must be completed and signed. Also, all kindergarteners are required to turn in a vision exam. Physicals with updated immunizations and kindergarten vision exams must be turned into the nurses office by October 1st of the current school year. In accordance with state law, a student that is not in compliance with these mandates on October 15th will be excluded from school until all required health information has been received.
OUT OF STATE TRANSFER: All students enrolling for the first time in an Illinois public school are required by the state to have a current physical examination and immunizations completed on the approved Illinois Department of Public Health Form due by October 1st. For students transferring after the start of the year, all health documentation is due 30 days after the start date.
IN STATE TRANSFER: All students transferring to District 35 from another Illinois public school are required to submit documentation of a physical examination and immunizations completed on the approved Illinois Department of Public Health Form due by October 1st. For students transferring after the start of the year, all health documentation is due 30 days after the start date.
DENTAL EXAMINATION
All students entering kindergarten, second and sixth grade grades are required to have a dental examination. Dental examinations are due by May 15th of the year the student is enrolled in kindergarten, second or sixth grade. A current examination is one that was completed within 18 months of May 15th.
EYE EXAMINATION REQUIREMENTS
All students enrolling in a kindergarten or enrolling for the first time in an Illinois public school shall submit proof of an eye examination completed within the previous year before October 1st of the school year.
